Where to view Patient Forms

You can send patients forms to fill out. Once they have filled them out, you can then view them.

However, how and where can you view patient forms?

You can access forms that patients have already filled out in two places.

  1. Under Patient Forms in HelloHealth; and
  2. Under Documents in PractiseSuite.

Forms in HelloHealth are sent back to the provider immediately after they have been filled out. Once the patient clicks on ‘Submit’, the provider will access this form from any of the two mentioned places.

Where to view Patient forms in HelloHealth?

Providers can share forms with patients automatically when an appointment is booked, or manually via messages.

When forms are part of the appointment details, once patients have completed and submitted the forms, they can’t edit them again.

This means that the provider will have access to these forms under the ‘Patient Forms’ tab on HelloHealth.

Where to view Patient Forms

There are filters available for providers to use to comb through the different forms and you can filter them by:

  1. Type – You can select whether you want to view appointment forms, message forms, or both.
  2. Provider
  3. Date Filled – You can select the exact date containing the forms you would like to see.
  4. Age Group – By selecting this filter, you can set the age group that has the forms you would like to view

Additional filters include the patient’s first name, last name, email address, and phone number.

Once on the Patient Forms screen, you will see the list of forms and the patient(s) who completed the form(s).

To view the form, click on the icon on the right side of the screen and click on the . To download the form click on the icon, and the form will be saved on your device to view at a later stage or be uploaded elsewhere.

Forms In PracticeSuite

Similarly, once forms are completed by the patient, they will be available for you to view in PracticeSuite.

The forms will be located under the documents section of the patient’s details in PracticeSuite for you to view.
